RMN offers both in-person and telehealth appointments. In-person appointments take place at our Huntsville office:

Referrals

Alabama law requires a doctor’s referral for insurance based nutrition therapy, even though your insurance company may not require one. Alabama clients will need to make this request, though doctors are typically happy to provide one. You can obtain a referral from your primary, or specialist doctor, nurse practitioner, or PA, or from a Psychiatrist. RMN will assist you with obtaining the referral if needed. 

Fees and Insurance

RMN is in network with several major insurance companies. As you might know, navigating the insurance world can be tricky. 

Please consult your insurance company’s member services to verify that you are covered for Medical Nutrition Therapy (MNT) . You can check using the following CPT codes: 97802, 97803, and diagnostic code Z71.3.

RMN will also work to verify your coverage prior to your appointment. We cannot, however, guarantee that an appointment will be covered. We will try hard to push back against any insurance refusals, but If it ends up that your visit is not covered, you will be responsible for the cash price.

Keeping a credit card number on file streamlines this process. The card number is held confidentially with Kalix, our electronic medical record software, and Rachael does not keep a separate record of, or have access to it. If your insurance claim is denied, your credit card will be billed for the cash price.

Clients without insurance are welcome as well! Credit card, Venmo, Check and cash are accepted.

Appointment durations are as follows:

• Initial 75 mins
• Follow-up 55 mins

Medicare

Because Rachael does not accept Medicare, she offers a discounted senior cash rate to those individuals on Medicare.  Please contact us  for information.

Missed and Cancelled Appointments 

Your appointment time is reserved for you, and Rachael Martin Nutrition will never double book a client. Because of this and to be gracious to those on the waiting list, the following fees will apply:

Appointments cancelled within 48 hours of your appointment will result in a $75 fee.

Last minute cancellations (within the hour) and no-show appointments will be charged the full cash amount for a scheduled appointment. 

True emergencies will be considered on a case by case basis and fees may be waived at the discretion of RMN.
